This form places hard copy in the hand of the prospect indicating the date of his or hers profession of faith.  

It also furnishes a place to put the date of their Baptism and Church Membership and at the same time gives the Worker a chance to say something about them.  

This is not as important as some may think, but it has proven valuable to some degree in the past.

A group of older teens were making their way to our booth. I picked out the one I thought was vocal and the rest might follow. As they approached I ask the chosen boy if he had seen the 3 Things God Cannot Do?
He said, "Yes, last year."
I asked, "Do you know for sure that you have been born again?"
He said, "Yes (without hesitation), just a minute."
He then pulled out his billfold and started pulling stuff out of it. Finally he pulled out a Birth Certificate Form with his name on it and last year's date.
Then he said, "I got saved last year in this very same place (booth)".
He was not ashamed and recommended that the others sit and listen.

On Thanksgiving Day a pastor who obtains several booths for us in south Texas called us. He went on to say he had just come from a home where an older teen boy had succumbed during a seizure. The parents were shocked. He had never had a seizure before.

The pastor went on to tell us that the parents had gone through his billfold and found a curious piece of paper (Birth Certificate Form) and wanted to know what it was about.

The pastor went on to relate how the boy must have made of profession of faith in Jesus Christ on the indicated date and that if he meant the profession that he had been saved from all his sins, past and future.

The parents were taken aback by all of this and were much comforted. They did not make a profession of faith, but their daughter did at the funeral service.

All in the family were much encouraged and received a good witness through the use of the Birth Certificate.
